NAFLD (now termed MASLD — metabolic dysfunction-associated steatotic liver disease) currently has two FDA-approved drugs for MASH with moderate-to-advanced (F2–F3) fibrosis: resmetirom (Rezdiffra, approved March 2024) and Wegovy (semaglutide 2.4 mg, approved August 2025). Other GLP-1 medications are NOT FDA-approved for MASLD itself, though they show peer-reviewed evidence for liver-fat reduction; their use here is off-label OR on-label for coexisting obesity / T2D.

Off-label considered options
Off-label use: Cite supporting evidence for off-label GLP-1 use in NAFLD/MASLD: semaglutide phase-2 trial (NEJM 2021, doi 10.1056/NEJMoa2028395) — significant NASH resolution at 72 weeks. Tirzepatide SYNERGY-NASH trial (NEJM 2024) showed even larger effect.
Non-pharmacologic alternatives
Weight loss (foundation)
7–10% body-weight loss can reverse hepatic steatosis. Mediterranean diet + 150 min/week exercise are first-line per AASLD.
Resmetirom (Rezdiffra)
FDA-approved March 2024 for NASH with fibrosis (F2–F3) — the first on-label drug for advanced MASLD/NASH, joined by Wegovy (semaglutide 2.4 mg) in August 2025.
Vitamin E (selected patients)
AASLD-recommended for biopsy-confirmed NASH in non-diabetic adults; not for all NAFLD.
