Metabolic dysfunction-associated steatotic liver disease (MASLD, formerly NAFLD) affects 25-30% of US adults. GLP-1 and GLP-1/GIP agonists show robust reductions in liver fat. Wegovy (semaglutide 2.4 mg) was FDA-approved in August 2025 for MASH with moderate-to-advanced (F2–F3) liver fibrosis — the first GLP-1 approved for a liver disease — based on the phase 3 ESSENCE trial (MASH resolution and fibrosis-improvement endpoints met). Resmetirom (Rezdiffra) was the first MASH-specific drug (approved 2024). Tirzepatide showed similar liver-fat and fibrosis improvements in SYNERGY-NASH but is not yet FDA-approved for MASH. Evidence grade: A (phase 3 RCT for semaglutide). Outside the Wegovy MASH indication, GLP-1s are used off-label or based on coexisting obesity/T2D indications.
Non-alcoholic fatty liver disease (NAFLD) — recently renamed MASLD (metabolic dysfunction-associated steatotic liver disease) — affects ~25% of US adults. It is the most common liver disease, strongly linked to obesity, T2D, and insulin resistance. ~20% of NAFLD patients develop NASH (steatohepatitis) which can progress to cirrhosis. GLP-1 medications reduce liver fat and inflammation, and in August 2025 Wegovy (semaglutide) became the first GLP-1 FDA-approved for MASH with moderate-to-advanced (F2–F3) fibrosis, based on the phase 3 ESSENCE trial.

Diagnostic algorithm
