Medications prescribed for prediabetes
Drugs prescribers may consider — discuss with your prescriber.
No GLP-1 medication carries an FDA indication for pure prediabetes (A1c 5.7–6.4%). However, Wegovy and Zepbound ARE FDA-approved for chronic weight management when BMI thresholds are met (≥30, or ≥27 with weight-related comorbidity such as prediabetes itself). Saxenda is also FDA-approved for weight management. The CDC Diabetes Prevention Program (DPP) and metformin (off-label) remain first-line.
FDA-approved options
Each carries an on-label FDA indication relevant to prediabetes. Efficacy data, monthly cost ranges, and prescriber considerations summarized below; tap a card for full drug review.
Off-label considered options
Off-label use: Saxenda is FDA-approved for weight management in adults with BMI thresholds — this can apply to a prediabetes patient who also meets weight criteria. Use of GLP-1s purely to delay T2D progression (without weight criteria) is OFF-LABEL.
Non-pharmacologic alternatives
Diabetes Prevention Program (DPP)

Metformin (off-label)
Reduces T2D progression by ~31%. Best for BMI ≥35, age <60, or gestational diabetes history.
Lifestyle changes alone
Targeted diet (low-glycemic, Mediterranean) + 150 min/week exercise for lower-risk patients.
