GLP-1 prices by US state

The same Wegovy or Zepbound prescription can cost dramatically different amounts depending on where you live — even with the same drug, same dose, and same manufacturer programs. Three factors drive the difference:

1. State telehealth rules

About 14 states require synchronous video consultation for new GLP-1 prescriptions; the rest permit asynchronous (chat / form-based) telehealth. Video-only states tend to have higher costs because fewer providers serve them and the consultation fee is higher.

2. Medicaid coverage differential

States with comprehensive Medicaid GLP-1 coverage (California, Massachusetts, Pennsylvania, Virginia, Minnesota) generally see slightly higher self-pay averages — because the cheapest segment of the market is absorbed by Medicaid. States without Medicaid coverage (Arkansas, Mississippi, Idaho, Missouri) push more demand into the cheapest cash-pay tier, pulling the average down.

3. Compounded vs brand mix

States with high obesity rates and lower household income (the Deep South, parts of Appalachia) have more active compounded-pharmacy advertising.
