- Dosing frequency
- Once daily, in the morning, on an empty stomach. Swallow whole with ≤4 oz of plain water; wait at least 30 minutes before any food, drink, or other oral medications.
Titration timeline
FDA-labeled step-up schedule. Most clinicians follow this exactly, but may pause at a step for an additional 4 weeks if side effects are intolerable.

Initiation dose — not a therapeutic dose. Used to improve GI tolerability.

If a step-up is poorly tolerated
Hold the current dose for an additional 4 weeks if the step-up is poorly tolerated. Many patients achieve target A1c on 7 mg without needing 14 mg.
Missed-dose protocol

Same day only
If a dose is missed, skip it entirely and take the next dose the following morning at the usual time. Do NOT take an extra dose later in the day to compensate — the absorption window depends on the empty-stomach protocol.
When to contact your prescriber
- Multiple consecutive missed doses.
- Inability to maintain the empty-stomach dosing protocol.
- Severe persistent GI symptoms.
