Wegovy dose escalation schedule
Week-by-week titration for Wegovy (semaglutide) — starting dose, step-up schedule, maintenance, maximum, and what to do if you miss a dose.
Starting dose
0.25 mg once weekly
Maintenance
2.4 mg once weekly (recommended) or 1.7 mg once weekly
Maximum dose
7.2 mg once weekly (Wegovy HD), for adults losing weight who have tolerated 2.4 mg for at least 4 weeks
Dose form & administration
- Dose form
- Once-weekly injection (single-dose pen, single-dose syringe, or the four-dose FlexTouch pen), and a once-daily tablet
- Dosing frequency
- Once weekly, same day each week, any time of day, with or without meals
Titration timeline
FDA-labeled step-up schedule. Most clinicians follow this exactly, but may pause at a step for an additional 4 weeks if side effects are intolerable.

If a step-up is poorly tolerated
If a dose is not tolerated during escalation, the label says to consider waiting 4 more weeks before the next increase.
Missed-dose protocol

Next dose more than 2 days away
If your next scheduled dose is more than 2 days (48 hours) away, inject the missed dose as soon as possible. If it is less than 2 days away, skip it and inject on your usual day.
When to contact your prescriber
- You missed 2 or more doses in a row: the label says to restart the escalation at a lower dose.
- Nausea or vomiting keeps you from moving up to the next dose.
- You are pregnant or planning a pregnancy.
