Do GLP-1 medications raise gallbladder risk? Yes, modestly, and it's on every label
Yes. GLP-1 receptor agonists are associated with a modestly increased risk of gallstones and gallbladder disease, and this is not a rumor from social media. It appears in the FDA-approved prescribing information for every drug in the class and is backed by a large meta-analysis of randomized trials.
The strongest single piece of evidence is a 2022 systematic review and meta-analysis published in JAMA Internal Medicine (He et al.). Pooling 76 randomized clinical trials covering 103,371 patients, the authors found that being randomized to a GLP-1 receptor agonist was associated with a 37% higher relative risk of the combined outcome of gallbladder or biliary disease (relative risk 1.37; 95% confidence interval 1.23-1.52). In absolute terms, that worked out to roughly 27 additional events per 10,000 patients per year.
Broken down by condition, the same analysis found higher risks of gallstones (cholelithiasis, RR 1.27), gallbladder inflammation (cholecystitis, RR 1.36), broader biliary disease (RR 1.55), and gallbladder-removal surgery (cholecystectomy, RR 1.70). Reassuringly, it found no statistically significant increase in biliary tract cancer.
The word 'modest' matters. A 37% relative increase sounds large, but because gallbladder disease is uncommon to begin with, the absolute increase for any one person is small. That is why clinicians describe the risk as real but uncommon, not a reason most eligible patients avoid these medications.
What the FDA labels actually warn (Ozempic, Wegovy, Zepbound and more)
Every GLP-1 sold in the U.S. carries a Warnings and Precautions section titled 'Acute Gallbladder Disease.' The exact wording and reported rates differ by product, and the drugs approved specifically for weight loss (higher doses) tend to report higher numbers than the lower diabetes doses.
Ozempic (semaglutide injection) states that acute gallbladder events such as gallstones or cholecystitis have been reported in trials and after marketing; in placebo-controlled trials, gallstones were reported in 1.5% and 0.4% of patients on the 0.5 mg and 1 mg doses and in none of the placebo patients (Novo Nordisk prescribing information, 2025).
Wegovy (semaglutide 2.4 mg for weight loss) reports gallstones in 1.6% of treated adults versus 0.7% on placebo, and cholecystitis in 0.6% versus 0.2%. Its label adds a notable point: the increase held 'even after accounting for the degree of weight loss,' suggesting the drug contributes beyond rapid weight loss alone (Wegovy prescribing information, 2025).
Zepbound (tirzepatide) reports gallstones in 1.1% of treated patients versus 1% on placebo, cholecystitis in 0.7% versus 0.2%, and gallbladder removal in 0.2% versus none, noting gallbladder events were associated with weight reduction (Zepbound prescribing information, 2025). Rybelsus (oral semaglutide) reports gallstones in about 1% of patients on the 7 mg dose (Rybelsus/DailyMed label, updated October 2025). All labels advise that if gallstones or cholecystitis are suspected, gallbladder imaging studies are indicated.

Why GLP-1s affect the gallbladder: two overlapping mechanisms
Researchers point to two overlapping reasons, and it is often hard to separate them.
First, rapid or substantial weight loss itself is a well-established trigger for gallstones, independent of any drug. When the body burns fat quickly, the liver puts out more cholesterol into bile, which can tip bile toward forming stones. This is the same reason gallstones are a recognized risk after bariatric surgery and very-low-calorie diets. Because GLP-1s can drive large, fast weight loss, some of the gallbladder risk simply travels with the weight loss.
Second, GLP-1s appear to slow gallbladder emptying. These drugs slow the movement of the gut, and evidence suggests they also reduce how briskly the gallbladder contracts and empties. When bile sits in the gallbladder longer and becomes more concentrated (a state called biliary stasis), stones are more likely to form. The JAMA meta-analysis supports a drug-specific contribution: risk rose with higher doses and with treatment beyond 26 weeks, and Wegovy's label reports excess gallbladder disease even after adjusting for weight loss, which points to more than weight change alone.
Warning signs of a gallbladder attack: when to get help
Most people on GLP-1s never develop a gallbladder problem, but everyone taking one should recognize the warning signs, because acute cholecystitis and a blocked bile duct are medical emergencies. GLP-1 labels instruct that if gallstones or cholecystitis are suspected, gallbladder imaging and clinical follow-up are indicated, so these symptoms are worth prompt attention.
Do not start, stop, skip, or change the dose of any GLP-1 on your own based on symptoms. Whether to pause or continue a medication during a gallbladder workup is a decision for the prescribing clinician, who can weigh your full picture.
Who may be at higher risk, and questions for your clinician
Some factors are associated with higher baseline gallstone risk regardless of GLP-1 use, and they may compound the picture. Well-recognized risk factors include female sex, older age, obesity, a family history of gallstones, a prior history of gallstones, rapid weight loss, and pregnancy. The JAMA analysis specifically found risk was higher at the larger doses used for weight loss and with longer treatment duration.
This does not mean people with these factors cannot take GLP-1s. It means the risk conversation is individual. A clinician can weigh your personal history against the benefits of treatment, decide whether any monitoring makes sense for you, and tell you what symptoms should prompt a call.

Compounded semaglutide and tirzepatide are different: as compounded preparations, they are not FDA-approved finished drugs and have not gone through the same testing for safety, purity, and consistency.
That means the gallbladder-risk figures from the brand labels are the best evidence available, but they were generated with the approved products, not with compounded versions.
