What is fatty liver disease (MASLD/MASH)?
Fatty liver disease — now called MASLD (metabolic dysfunction-associated steatotic liver disease, formerly NAFLD) — is excess fat in the liver tied to weight, insulin resistance, and metabolic syndrome. Its inflammatory, scarring form is MASH (formerly NASH), which can progress to fibrosis and cirrhosis. It’s strongly driven by the same factors GLP-1 medications target.
Do GLP-1s treat fatty liver?
The evidence is now strong. In the phase-3 ESSENCE trial, 72 weeks of semaglutide achieved MASH resolution without worsening fibrosis in about 63% of patients — leading to FDA approval of Wegovy for MASH (with F2–F3 fibrosis) in August 2025, only the second drug ever approved for the condition. Tirzepatide’s SYNERGY-NASH trial reached similar resolution rates (up to ~62% at the highest dose). Improvement tracks with weight loss.
Which GLP-1 for fatty liver?
Semaglutide (Wegovy) carries the specific FDA MASH indication; tirzepatide (Zepbound) has comparable trial data and the most weight loss overall.
