Metformin vs Ozempic at a glance
Metformin and Ozempic treat the same disease from opposite ends of the toolkit. How each drug works (mechanism)
Metformin and Ozempic lower blood sugar through completely different biology. Metformin is a biguanide: its main effect is reducing the amount of glucose the liver produces and making the body's tissues more sensitive to insulin (StatPearls/NCBI; ADA Standards of Care, 2025). It does not meaningfully suppress appetite or slow digestion, which is why its weight effect is small.
Ozempic (semaglutide) is a GLP-1 receptor agonist. It mimics the natural gut hormone GLP-1: it prompts the pancreas to release insulin when blood sugar is high, slows how fast the stomach empties, and reduces appetite by acting on appetite centers in the brain (Ozempic FDA label, 2025). Those appetite and gastric-emptying effects are what make GLP-1 drugs produce much larger weight loss than metformin.
Weight loss: Ozempic produces substantially more
For weight loss specifically, the gap is large and consistent across trials. Metformin is considered weight-neutral with the potential for modest loss — typically around 2-3% of body weight, and in some people none at all. In the long-running Diabetes Prevention Program, metformin produced about 2.1 kg of weight loss versus 5.6 kg for intensive lifestyle change over roughly three years (DPP; NEJM, 2002).
Ozempic produces far more. In type 2 diabetes trials, semaglutide reduced weight by roughly 6-7% — about 13 lbs (6.0 kg) at the 1 mg dose and about 15 lbs (6.9 kg) at the highest 2 mg dose over about 40 weeks (Ozempic FDA label, 2025; SUSTAIN FORTE, Lancet Diabetes & Endocrinology, 2021). An important honesty point: Ozempic itself is not FDA-approved for weight loss. The same molecule at a higher dose is sold as Wegovy (semaglutide 2.4 mg) specifically for chronic weight management, and in the STEP 1 trial Wegovy produced an average 14.9% weight loss over 68 weeks, versus 2.4% for placebo (New England Journal of Medicine, 2021).
Bottom line: if the primary goal is weight loss, GLP-1 medications like Ozempic/Wegovy have produced several times the weight loss of metformin in trials. That does not make them right for everyone — it makes the efficacy difference real, and worth weighing against cost, side effects, and a clinician's judgment.
Cardiovascular and kidney benefits
Ozempic carries FDA-approved cardiovascular and kidney indications that metformin does not. In the SUSTAIN-6 cardiovascular outcomes trial (3,297 adults with type 2 diabetes and established cardiovascular disease, over ~2 years), semaglutide reduced the risk of major adverse cardiovascular events — cardiovascular death, non-fatal heart attack, or non-fatal stroke — by 26% versus placebo (hazard ratio 0.74; 95% CI 0.58-0.95) (SUSTAIN-6, NEJM, 2016). On that evidence, Ozempic is FDA-approved to reduce MACE in adults with type 2 diabetes and established heart disease, and to reduce the risk of worsening kidney function in those with type 2 diabetes and chronic kidney disease (Ozempic FDA label, 2025).
Metformin has a long track record and is inexpensive and well-studied, but it does not carry the same trial-proven, FDA-approved cardiovascular-event-reduction claim. Whether cardiovascular protection is a deciding factor depends heavily on an individual's heart and kidney history — a clinical assessment, not a self-diagnosis.
Cost, form, and convenience
Cost and convenience run in metformin's favor. Metformin is a generic oral tablet taken once or twice daily and typically costs about $4-$15 per month (widely reported generic pricing). Ozempic is a once-weekly self-injected pen with a list price around $900-$1,000 per month; what a person actually pays depends heavily on insurance coverage, deductibles, and manufacturer savings programs.
There is a tradeoff in the other direction: many people find a once-weekly injection simpler to remember than a daily pill, and Ozempic has no meaningful daily GI ramp-up once past the dose-escalation phase. But on raw affordability, metformin is dramatically cheaper, which is part of why guidelines still position it as first-line for many people with type 2 diabetes.
Side effects and safety
Both drugs are best known for gastrointestinal side effects, but the details differ. Metformin's most common side effects are diarrhea, nausea, and stomach upset, which often improve over time or with extended-release formulations. Long-term metformin use can lower vitamin B12 absorption, so B12 levels are sometimes monitored (Diabetes, Obesity & Metabolism, 2024). Metformin carries a boxed warning for lactic acidosis — a rare but serious complication, more likely in people with significant kidney impairment or other risk factors.
Ozempic's most common side effects are also gastrointestinal — nausea, vomiting, diarrhea, and constipation — usually most noticeable during dose escalation and often easing with time (Ozempic FDA label, 2025). Its label carries a boxed warning about thyroid C-cell tumors seen in rodent studies; it is contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2. GLP-1 drugs have also been associated with gallbladder problems and, rarely, pancreatitis. Neither drug's side-effect profile can be self-assessed — tolerability and contraindications are for a prescribing clinician to evaluate.
When each is used — and can they be combined?
Metformin remains the historically standard first-line medication for type 2 diabetes for most adults, largely because it is effective, inexpensive, and well-tolerated (ADA Standards of Care in Diabetes, 2025). Ozempic is generally considered when additional blood-sugar control is needed, when weight loss is a priority, or when a person has established cardiovascular or kidney disease that Ozempic is proven to help.
Importantly, this is not strictly either/or.
