Does Ozempic cause bloating?
Yes. Ozempic is semaglutide, and semaglutide slows gut transit, so food moves through more slowly and has more time to ferment into gas. Slower motility plus trapped gas is what produces the distended, bloated feeling — unpleasant but not dangerous on its own.
In the trials, bloating was reported in belching and gas in about 7% of patients, with bloating and abdominal distension commonly reported alongside them. Those figures come from the weight-management trials of semaglutide; Ozempic is a lower (diabetes) dose, so the rates you personally experience may be lower — but the mechanism and the management are the same.
When it starts and whether it fades
Tends to be worst when transit is slowest — early on and after each dose increase — and eases as the gut adapts. Often worse after a large meal or gas-forming foods.
What actually helps
- Smaller meals eaten slowly, and limiting gas-forming foods — beans, onions, carbonated drinks, and cruciferous vegetables like broccoli and cabbage — is the change people report helps most.
- Gentle movement after meals (a short walk) helps gas move through. Simethicone, an over-the-counter anti-gas remedy, is a reasonable option — check with your pharmacist.
- If bloating comes with constipation, treating the constipation (fluids, fiber, an osmotic laxative if needed) often relieves the bloating too.
These management steps are general gastroenterology practice, not instructions from the Ozempiclabel — there is no semaglutide-specific management guideline. Run any laxative, anti-diarrheal or supplement past your prescriber or pharmacist, especially alongside other medications.
When it is an emergency, not a side effect to manage
On its own, bloating is benign. Bloating combined with vomiting, a distended, swollen abdomen and no bowel movement is the combination to worry about — it can be an early sign the gut has stalled (ileus), which the label lists as a reported reaction and is an emergency.
