Why this comparison is tricky
Mounjaro and Wegovy get compared because both are famous once-weekly shots that cause big weight loss. But they're approved for different things: Mounjaro treats type 2 diabetes, Wegovy treats obesity. That means their weight-loss numbers come from different trials with different participants, so putting “22% vs 15%” side by side isn't a fair fight. To compare fairly, you either match the indications or look at a direct head-to-head.
The fair comparison: SURMOUNT-5
The SURMOUNT-5 trial (NEJM 2025) tested the two molecules directly in people with obesity: tirzepatide −20.2% vs semaglutide −13.7%over 72 weeks. That's the real answer to “which loses more weight” — but note it compared the weight-loss brands (Zepbound vs Wegovy), since Mounjaro itself isn't a weight-loss product.
The indication and coverage split
Because Mounjaro is a diabetes drug and Wegovy a weight-lossdrug, insurance often treats them very differently. Many plans cover diabetes medications more readily and put up more barriers to — or entirely exclude — weight-loss drugs. - Your goal is weight loss: compare Zepbound vs Wegovy (both weight-loss approved, with a real head-to-head trial).
- Coverage is the constraint: a diabetes diagnosis usually makes the diabetes-branded option easier to obtain.

Almost every confusing GLP-1 comparison dissolves once you see the grid. There are two active drugs, and each is sold under one brand for diabetes and another for weight — same molecule, different product, different label, different coverage.

Comparing Mounjaro to Wegovy compares across both axes at once — a different molecule and a different indication — which is why it produces no clean answer. Compare down a column (same purpose) or across a row (same molecule), and the question becomes answerable.
Why the brand you're prescribed decides your cost
The molecule is not what your insurer prices — the product is. That has three consequences worth knowing before you ask for a specific brand:
- Diagnosis drives coverage. A diabetes brand prescribed for diabetes is ordinary covered care. The same molecule prescribed for weight, under the weight brand, is covered only if your plan includes weight-loss drugs — and many employer plans exclude them outright.
- Off-label prescribing rarely helps financially. Getting a diabetes brand for weight loss is off-label, which usually means paying cash — the coverage advantage evaporates precisely because the indication no longer matches.
- Non-weight indications can bypass an exclusion. Wegovy carries a cardiovascular-risk-reduction indication and Zepbound a sleep-apnea indication. Those are not weight-loss uses, so coverage can apply where a blanket weight-loss exclusion would block it.
