Why this is the wrong matchup
Zepbound and Ozempic get compared because both are blockbuster once-weekly shots — but they are different molecules approved for different things, at different doses. Ozempic is tuned for blood-sugar control in diabetes; Zepbound is tuned for weight loss. So when you see “Ozempic only causes ~6 kg of weight loss,” that's from diabetestrials at diabetes doses — it isn't what the semaglutide molecule does when dosed for weight loss (that's Wegovy). Zepbound versus Ozempic crosses both axes — different molecule and different indication — which is why it has no clean answer. Compare down a column (same purpose) or across a row (same molecule) and the question becomes answerable.
Why the brand decides your cost, not the molecule
Insurers price products, not molecules. Three consequences worth knowing before you ask for a particular brand:
- Your diagnosis drives coverage. A diabetes brand for diabetes is ordinary covered care. The same molecule under the weight brand is covered only if your plan includes weight-loss drugs — and many employer plans exclude them entirely.
- Off-label rarely saves money. Asking for a diabetes brand to treat weight is off-label, which usually means paying cash — the coverage advantage disappears exactly because the indication no longer matches.
- A non-weight indication can bypass an exclusion. Zepbound carries a sleep-apnea indication and Wegovy a cardiovascular-risk-reduction indication; neither is a weight-loss use, so coverage can apply where a blanket exclusion would otherwise block it.
