Door 1: the weight-loss criteria
| Requirement | What satisfies it |
|---|---|
| BMI ≥30, or ≥27 + a condition | Chart-documented BMI; if 27–30, a diagnosed comorbidity (hypertension, T2D, dyslipidemia, OSA, or CVD) |
| Documented lifestyle program | Supervised diet-and-exercise records, commonly ~90 days, with dates |
| Baseline weight | Recorded at initiation — the number your ≥5% renewal is measured against |
| Possible Wegovy step | If required: documented 12–16 week Wegovy trial with <5% loss or intolerance |
| Renewal: ≥5% loss | Documented ≥5% weight loss from baseline at ~4–6 months |
Door 2: the sleep-apnea path
In December 2024, Zepbound became the first medication FDA-approved for moderate-to-severe obstructive sleep apnea in adults with obesity. Because OSA is a medical indication — not weight loss — this route behaves completely differently:
- Plans that exclude weight-loss drugs may still cover Zepbound for OSA.
- Medicare Part D, barred by law from weight-loss coverage, can cover it for OSA.
- Documentation is typically a sleep study showing moderate-to-severe OSA (commonly AHI ≥15) plus obesity — often a shorter path than assembling 90 days of lifestyle records.
If you have (or suspect) sleep apnea, raise this with your prescriber before filing a weight-loss PA. See Zepbound for sleep apnea.
Which door should you use?
You have documented moderate-to-severe OSA
Use the OSA indication — it sidesteps the weight-loss exclusion entirely and usually needs less documentation.
Your plan covers weight-loss drugs, no OSA
Use the weight-loss criteria; expect a possible Wegovy step first.
Your plan excludes weight-loss drugs, no OSA
A PA won't help. Check the Medicare Bridge (if on Medicare) or cash-pay from ~$299/mo.
You're on Medicare
Two options: the OSA indication, or the GLP-1 Bridge ($50/mo for the Zepbound KwikPen).
Denied? Match the fix
| Denial reason | Fix |
|---|---|
| Step therapy (Wegovy first) | Document the 12–16 week Wegovy trial outcome or intolerance |
| Missing documentation | Submit baseline weight, lifestyle records, comorbidity diagnosis |
| Weight-loss drugs excluded | Pivot to the OSA indication (sleep study), the Medicare Bridge, or cash-pay |
| Non-preferred / non-formulary | Request a formulary exception (medical necessity) |
Steps and deadlines: denial appeal · formulary exception · cross-drug PA pillar · Zepbound insurance coverage · Medicare GLP-1 Bridge.
