Semaglutide compounding pharmacies: who makes it, and what is on their record
The telehealth brand you buy from is not the pharmacy that compounds the drug. Here is who does, what each one's licence and published regulatory record says, and why the price argument for compounded semaglutide has largely stopped working.
The short answer
- 10 pharmacies fill most US compounded semaglutide prescriptions. The telehealth brand on the website is not one of them.
- Oral Wegovy is $149/month at 1.5 mg and 4 mg and is FDA-approved. Compounded semaglutide runs roughly $99-249. The ranges now overlap, so the discount for choosing an unreviewed product has largely gone.
- One date to diary: the $149 price for 4 mg runs only to 31 August 2026, after which 4 mg becomes $199/month.
- No compounded semaglutide is FDA-approved. The agency does not review compounded preparations for safety, effectiveness, potency or purity — this applies to every pharmacy on this page equally.
- The shortage that justified mass compounding ended in February 2025. Patient-specific compounding continues where a prescriber documents a clinical reason; price is not one.
- Distance is irrelevant. These are mail-order pharmacies shipping nationwide, and "near me" tells you nothing about the thing you should be checking.
Why the price argument stopped working
Compounded semaglutide existed as a mass-market product because the brand was unaffordable and unavailable. Both halves of that changed. Novo Nordisk now sells an oral semaglutide tablet direct at a price that sits inside the compounded range.

Compare the top row with the bottom one. A reader choosing compounded semaglutide in 2026 on price alone is, at the lower doses, paying about the same for a product no regulator has examined. That may still be the right choice for someone whose prescriber has a clinical reason for it. It is no longer a bargain.

Where compounded semaglutide stands legally
The FDA declared the semaglutide shortage resolved in February 2025, which removed the basis on which most mass compounding had been happening. A 503A pharmacy can still compound for an individual patient where the prescriber documents a clinical reason the commercial product does not meet. Some pharmacies add a second ingredient — vitamin B6 is the common one — and argue the result is therefore not "essentially a copy" of the approved drug. That argument has not been settled for every formulation, and on 20 February 2026 the FDA sent roughly 30 warning letters in a single day to telehealth companies over how compounded GLP-1s were described to buyers.

Usually not any more, and this is the fact most pages on the subject have not updated. Oral Wegovy is $149/month for the 1.5 mg and 4 mg strengths direct from Novo Nordisk, and it is FDA-approved. Compounded semaglutide programmes advertise roughly $99 to $249 a month depending on the provider and the plan. So the ranges overlap, and at the lower doses the approved tablet is competitive or cheaper. Note one date: the 4 mg price of $149 runs only to 31 August 2026, after which 4 mg moves to $199.
Is compounded semaglutide FDA-approved?
No. The FDA approves finished drug products and does not review compounded preparations for safety, effectiveness, potency or purity. That is true of every compounded semaglutide regardless of which pharmacy prepares it, and it is not a comment on any individual pharmacy.
Is compounding semaglutide still legal?
The basis narrowed. Mass compounding of copies rested largely on semaglutide being on the FDA shortage list; FDA declared that shortage resolved in February 2025. A 503A pharmacy can still compound for an individual patient where a prescriber documents a clinical reason the commercial product does not meet — an allergy to an inactive ingredient, or a strength the approved product does not come in. Some pharmacies add a second ingredient such as vitamin B6 and argue the result is not "essentially a copy". Whether that reasoning holds has not been settled for every formulation.
How do I check the pharmacy my provider uses?
Ask in writing before paying: "Which compounding pharmacy fills my prescription, and is it a 503A or a 503B facility?" Then search the FDA warning-letter database under the LEGAL entity rather than the brand, check the FDA register of 503B outsourcing facilities if 503B is claimed, and look the licence up on the state board of pharmacy site. Ten minutes, and it is the only part of this that is genuinely verifiable.
