The four criteria that actually matter
1. Total monthly cost, not the headline
Add the membership/visit fee to the medication price — and price it at your maintenance dose, not the intro month. A "$0 visit" with a medication markup can cost more than a modest fee plus manufacturer cash pricing. The separated fee is the most common hidden-cost surprise in this category.
2. Insurance billing + prior-authorization help
If you have coverage, a provider that bills insurance and files the PA can cut your cost dramatically — Wegovy commonly needs one. If they are cash-only, you pay self-pay regardless of what your plan would have covered.
3. FDA-brand vs compounded
Some programs steer to compounded semaglutide (cheaper, not FDA-approved, now restricted post-shortage). If you want brand Wegovy, confirm it explicitly — the products are not interchangeable.
4. Ongoing dose management + support
Wegovy titrates over months. Good programs manage the step-ups, respond to side effects, and keep a clinician reachable — rather than disappearing after the first fill.
Total-cost worksheet
Programs quote different things, which makes them hard to compare. Fill in the same five lines for each provider and the picture clears up fast:
| Line item | What to ask for |
|---|---|
| Membership / visit fee | Monthly or annual — and whether it continues after the first month |
| Medication cost at maintenance dose | Not the starter/intro price — the dose you will actually settle on |
| Labs, if required | Whether bloodwork is required and who pays |
| Shipping | Included or extra |
| Commitment / cancellation | Any prepaid multi-month lock-in, and the refund policy |
For reference, the medication itself on Novo Nordisk's cash program is about $349/mo for maintenance doses at maintenance ($199/mo for the first 2 fills, then $349 intro). A program charging far more than that plus a modest fee is adding margin — see cheapest semaglutide.
Cheaper paths to check first
Before you pick a program, make sure you are not overpaying for the medication itself:
- Insurance — Wegovy is often covered with a prior authorization; see Wegovy PA criteria.
- Cardiovascular indication — if you have established heart disease, that route can win coverage where weight-loss drugs are excluded.
- Medicare GLP-1 Bridge — $50/month if you qualify.
- Savings card — Wegovy savings card for the commercially insured.
- TrumpRx — federal self-pay portal, pen from ~$199, pill from ~$149.
Red flags
No real clinician evaluation
Anything selling GLP-1s without a licensed-clinician assessment and a genuine prescription is unsafe and often illegal.
"Compounded Wegovy" wording
Wegovy is a branded product — there is no legitimate compounded Wegovy. That phrasing signals either confusion or misrepresentation.
Total cost hidden until checkout
If you cannot get the all-in maintenance-dose price up front, assume it is higher than advertised.
Long prepaid lock-ins
Multi-month prepayment with no refund is the most common source of complaints — check the cancellation policy first.
No named dispensing pharmacy
You should be able to learn which pharmacy fills it and whether it is accredited.
