Is Wegovy worth it? The real cost-vs-results math

For many people who qualify, yes — but it depends entirely on which price you pay. In trials, Wegovy produced about 15% average body-weight loss, and it's the first weight-loss drug proven to reduce cardiovascular events (heart attack, stroke, cardiovascular death) by 20% in people with established heart disease. Against that, the real cost is about $349/mo for maintenance doses self-pay — far less with insurance, or $50/month if you qualify for the Medicare Bridge. If it's covered or you can afford the cash price, the results are substantial; at full retail (~$1,349) indefinitely, the math is much harder.

In the STEP 1 trial, adults lost about 15% of their body weight on average over 68 weeks — roughly 30 pounds for someone starting at 200 lb. At about $349/month, a year costs roughly $4,200, which works out to around $140 per pound lost in that example. Results vary widely: some people lose considerably more, some less, and weight tends to return if you stop. The health benefits (blood pressure, blood sugar, and proven cardiovascular-risk reduction) are part of the value too, not just the scale.

It can be, at the manufacturer cash price (about $349/month maintenance, $199 introductory, and the oral version from about $149) — far below the ~$1,349 retail list. It is much harder to justify at full retail long-term. Before deciding, exhaust every cheaper path: insurance coverage (Wegovy is often covered with prior authorization, and its cardiovascular indication can open coverage where weight-loss drugs are excluded), the Medicare GLP-1 Bridge ($50/month if eligible), and the federal TrumpRx self-pay portal.

On raw effectiveness, Zepbound edges ahead — in the SURMOUNT-5 head-to-head trial, tirzepatide (Zepbound) produced 20.2% average weight loss versus 13.7% for semaglutide (Wegovy). On price they are close (Zepbound self-pay starts around $299/month, Wegovy around $349). Wegovy's advantage is its cardiovascular indication (valuable if you have heart disease, and a coverage route on plans that exclude weight-loss drugs); Zepbound's is greater weight loss plus a sleep-apnea indication. Both are reasonable — the tiebreakers are your conditions, coverage, and tolerance.

This is the honest catch: Wegovy is a long-term treatment, not a course. Weight typically returns after stopping, so the realistic budget is ongoing, not a one-time expense. At $349/month that is roughly $4,200 a year indefinitely; at the $50 Medicare Bridge price, about $600 a year. That ongoing-cost reality — more than the first-year math — is what should drive the decision, and it is why exhausting coverage options matters so much.

Wegovy may not be worth it if: you would pay full retail indefinitely with no path to coverage; you cannot tolerate the gastrointestinal side effects (nausea, constipation) that are common especially during dose escalation; you are looking for a short-term fix, since weight tends to return after stopping; or you do not meet the medical criteria (BMI ≥30, or ≥27 with a weight-related condition). For some people a different on-label option, or a different approach entirely, makes more sense — decide with a clinician.
