Case 1: you have type 2 diabetes
Here the value is strong, and it's about more than the A1c number:
Blood-sugar control
About a 1.5-1.8 point A1c reduction at the 0.5-1 mg doses across the SUSTAIN program, and up to ~2.1 points at the 2 mg dose. Reductions are larger from higher starting A1c.
Proven heart protection
SUSTAIN-6 showed a 26% reduction in major cardiovascular events in people with type 2 diabetes and high cardiovascular risk — an outcome benefit, not just a lab number.
Kidney protection
Ozempic carries an indication to reduce the risk of worsening kidney disease and cardiovascular death in adults with type 2 diabetes and chronic kidney disease (FLOW trial).
Usually covered
For diabetes, insurance commonly covers Ozempic with a prior authorization — which typically makes your copay the cheapest path available.

Case 2: you want it for weight loss
Using Ozempic just for weight loss is off-label, which has three practical consequences:
- Insurance rarely covers it for that use — so you pay cash.
- It's dosed for diabetes (max 2 mg), not weight management (Wegovy goes to 2.4 mg and was studied for weight).
- You lose the coverage routes Wegovy has — including its cardiovascular indication and the Medicare Bridge.
For weight loss the better-value option is almost always Wegovy — see also best telehealth for Wegovy.

The long-term reality
Ozempic is a chronic treatment. Blood sugar and weight benefits reverse if you stop, so the honest budget is ongoing — which is exactly why the covered-for-diabetes path matters so much more than any one-year cash calculation. If you have diabetes and coverage, the value is strong and sustainable. If you're paying full retail out of pocket indefinitely for an off-label weight-loss use, that's the scenario worth reconsidering.
Who should think twice
You want it only for weight loss
Off-label, rarely covered, dosed for diabetes — Wegovy is the better-value on-label choice.
You'd pay full retail long-term
~$969–$998/mo/month indefinitely is rarely sustainable — exhaust coverage, assistance, savings card, and TrumpRx first.
You can't tolerate GI side effects
Nausea and other GI effects are common during titration; if severe, the value equation changes.
